REMINDER
1. Each team must have 2 sets of numbered jerseys of different colours.
2. All participating teams must sign the CODE OF CONDUCT FORM and return it to the
Organizer during registration.
3. All participating teams must submit 9 Photostat copies of TEAM REGISTRATION FORM to
the Organizer during registration.
4. Each team must bring one new or painted approved ball for each half of the match.
5. Participants of the Under-12 category must bring along their respective Birth Certificate/
Identification Card or Photostat copy of Birth Certificate/ Identification Card (certified true copy by respective Headmaster / Headmistress) during registration.
6. Participants of the Under-15 and Under-18 category must bring along their respective Identification Card or Photostat copy of Identification Card (certified true copy by respective Headmaster / Headmistress) during registration.
7. Participants of the Varsity category must bring along their respective Matric Card or Certified true copy of Matric Card during registration.

10. The written protest together with a Protest Fee of THREE HUNDRED MALAYSIAN RINGGIT shall be handed to the Technical Officer who must record the time of receiving the
official protest form. The protest fee shall be forfeited if the protest is found to be frivolous.
11. KINDLY TAKE NOTE THAT THIS YEAR, ALL GAMES WILL BE PLAYED ON GRASS EXCEPT FOR VARSITY CATEGORY WHICH WILL BE ON TURF. HOWEVER, IF THE WEATHER OR OTHER FACTORS MAY NOT BE FAVOURABLE, GAMES SHALL BE PLAYED ON TURF. ONLY TURF SHOES WILL BE ALLOWED FOR MATCHES PLAYED ON THE TURF.
